 Puppy application This is an application/Questionnaire. In no means is this document to be construed as a guarantee of placement of puppy. This better enables us to find the right family and home for our babies. Please fill out form and remit via email to maryatchison@hughes.net No answer will necessarily exclude you from a puppy, so answer honestly. We will discuss every point with you. It is out mission to breed only the healthiest individuals. 3. Are you aware that there are some breed specific issues that are impossible to predict and or has nothing to do with the genetics.
